多认为该病毒首先侵入口腔粘膜和鼻粘膜在上皮组织中大量增殖后进入血循环(*次病毒血症)，jingxue流累及腮腺及一些组织，并在其中增殖再次进入血循环(第二次病毒血症)，并侵犯上次未受波及的一些脏器。病程早期时从口腔、呼吸道分泌物、血尿、乳汁、脑脊液及其他组织中可分离到腮腺炎病毒。有人分别人胎盘和胎儿体内分离出本病毒。根据本病患者在病程中可始终无腮腺肿胀而脑膜脑炎、睾丸炎等可出现于腮腺肿胀之前等事实，也证明腮腺炎病毒首先侵入口鼻粘膜jingxue流累及各种器官组织的观点。也有人认为病毒对腮腺有特殊亲和性，因此入口腔后即经腮腺导管而侵入腮腺，在腺体内增殖后再进入血循环形成病毒血症累及其他组织。各种腺组织如睾丸卵巢、胰腺、肠浆液造酶腺胸腺、甲状腺等均有受侵的机会，脑脑膜、肝及心肌也常被累及，因此流行性腮腺炎的临床表现变化多端脑膜脑炎是病毒直接侵犯中枢神经系统的后果，自脑脊液中可能分离出病原体。
腮腺的非化脓性炎症为本病的主要病变腺体呈肿胀发红，有渗出物，出血性病灶和白细胞浸润，腮腺导管有卡他性炎症，导管周围及腺体间质中有浆液纤维蛋白性渗出及淋巴细胞浸润，管内充塞破碎细胞残余及少量中性粒细胞腺上皮水肿、坏死、腺泡间血管有充血现象腮腺思周显著水肿，附近淋巴结充血肿胀。唾液成分的改变不多但分泌量则较正常减少。
